 Insurance Billing

More and more insurance plans are providing coverage for acupuncture treatments. But every insurance company is different. Please contact your insurance carrier to find out if your individual policy covers acupuncture. Use the Phone Questionnaire below as a step-by-step guide during the phone call.

Upon request, we will provide you with a SuperBill (a form that includes industry-standardized codes used by insurance companies).  You can submit this Superbill to your insurance carrier for reimbursement.

We do not guarantee reimbursement. Please note that payment is due at time of service by cash or check. If you have other questions or concerns, please do call us at (347) 706 0083 and let us assist you.

Alternatively, you can email your policy details to acupamela@gmail.com
